Upon arrival to the childcare center the Licensing Specialist observed 3 childcare personnel and 14 children mixed ages (Infant-VPK) sitting in the front room. This room has a capacity of 11 children. After the Licensing Specialist arrival, the childcare personnel split the children into 3 groups. During the time of the complaint inspection, the Licensing Specialist counted several times as the children lined up by the childcare personnel going back and forth to different classrooms. Once additional childcare personnel arrived the facility was back into compliance for insufficient staff/ratio. According to Florida Statutes (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed childcare facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one childcare personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one childcare personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one childcare personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one childcare personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one childcare personnel for every 20 children. 6. For children 5 years of age or older, there must be one childcare personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. During the time of the inspection, the Licensing Specialist observed that the childcare personnel for the (2) one-year olds and (6) two-year olds exited out of the building leaving her class unattended to turn parents/children away. After turning the parents in children away the childcare personnel then returned to her classroom. The childcare facility went back into compliance for inadequate supervision. According to the childcare facility handbook 2.4 Supervision 2.4.1 General Supervision Requirements B. Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location

During the time of the complaint inspection the Licensing Specialist observed that at least one former childcare personnel was never added to the employee roster. According to the childcare facility handbook 5 Background Screening 5.1 Initial Screening A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. K. The employer/owner/operator must add childcare personnel to their Employee/Contractor Roster in the Clearinghouse within 10 days of when the individual has received a childcare eligible result and has been hired at the facility. 41-01 · Technical assistance provided; owner stated she will contact the parent. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Childcare Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations
