41-02 · Immunization Records
During the time of the inspection the Licensing Specialist observed 4 children's files with expired immunization records. Per the DCF Handbook 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations

39-03 · Accident/ Incident Notification and Documentation
During the time of the inspection, the Director advised the Licensing Specialist that they did not have an accident/incident report on file for the bead incident. The Director submitted a completed accident/incident report via email on 6/4/24 to the Licensing Specialist. This violation was resolved and brought back into compliance. CCF Handbook, section 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. 30-14 · Diapering
During the time of inspection, the Licensing Specialist observed that two-year-old room did not have diapering procedures posted in the classroom. The standard was brought back into compliance by the Director by posting missing diaper procedures. Per DCF Handbook Section 3.10.2 Diapering Requirements L. Diaper changing procedure must be posted in the changing area and followed to protect the health and safety of children and childcare personnel

12-09 · Facility Environment
CCRC observed Live roaches crawling on the bathroom door and some were dead on the room floor in the seals room. Room was empty, children were moved to different classes until the issue is solved, director explained that this class was not in use since the parent brought the roaches issue to her attention. Director showed proof of treatment that was done on 07/21/2023. First technician sprayed base board only according to director, She called the company and asked for another Technician. Second technician came and sprayed stronger substance concentrating on the bathrooms. Director was told by technician to give 5 complete days for the roaches to die and to contact them again after. CCRC walked through the facility looking for evidence of vermin. Seals room was not free from evidence of, or presence of, vermin infestation. Provider is being given until 8/25/23 to bring this standard into compliance. 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months
