During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook: E. The employer/owner/operator must conduct employment history checks for substitutes, including documented attempts to contact each employer that employed the individual within the preceding five years and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. During the time of the inspection, the Licensing Specialist observed there to be blankets for nap time being stored on the shelf connected under the diaper Changing table leaving them accessible to cross-contamination. During the time of the inspection, this was resolved by the Operator removing the blankets and placing them in a closet. During the time of the inspection, the Licensing Specialist provided technical assistance: per the FDCH Handbook "B. Items unrelated to diaper changing shall not be stored in the diaper changing area nor shall they be placed on the diaper changing table."

During the time of the inspection, the Licensing Specialist observed there to be two diaper creams (Desitin and Boudreaux's Butt Paste) that had been administered without written consent from the custodial parent or guardian. During the time of the inspection this was resolved by the Operator removing the diaper creams from the daycare room to send home. During the time of the inspection, the Licensing Specialist provided technical assistance: per the FDCH Handbook "A. The operator must have written authorization from the custodial parent or legal guardian to dispense prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and must contain the childs name; the name of the medication to be dispensed; and the date, time and amount of dosage to be given. This record shall be initialed or signed by the child care personnel who gave the medication. Prescription and non-prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care personnel must never administer a medication that is prescribed for one child to another child.L. Use of diaper creams and insect repellant may only be utilized with written permission from parents/guardians. Manufacturer instructions should be followed."
