    During the time of inspection, Licensing Specialist observed that a 3-month-old infant was left alone inside the home in an infant chair while provider was outdoors with 7 other children (one year through four years) having lunch. Licensing Specialist observed Provider enter the home, pick infant up and bring her outside with the rest of the children. FDCH/LFCCH Handbook, section 6 Supervision A. The operator shall remain responsible for the supervision of the children in care and capable of responding to emergencies and the needs of the children at all times. Child care personnel must directly supervise children, both indoors and outdoors, by sight and sound. Children must never be left without child care personnel supervision inside or outside the home, in a vehicle, or at a field trip location by themselves. This standard was brought back into compliance during the time of inspection

    During the time of inspection, Licensing Specialist observed that the plastic gate still had broken panels. Provider advised that it was previously repaired, but the children broke it again.     During the time of inspection, the Licensing Specialist observed, in the outside play area a cable box that had exposed wires and a broken brick lying next to it. In one of the playhouses there was protruding nails from the roof of the house where children could potential be harmed. On the playground there was also a wooden playset that was repaired but exposed nails were rusty and protruding. There were also 2 scooters that had broken pieces on the handlebars that had rust on the edges. The wooden fencing had 2 wooden planks that needed repair and also a plastic gate that had broken panels. CCF Handbook:7.6 Outdoor Time, Fencing and Play Area Requirements Outdoor Play areas shall be clean, in good repair and free from litter, nails, glass, and other hazards

    During the time of inspection, the Licensing Specialist observed, 2 mats that ripped and torn with exposed foam. 7.8 Appropriate, Safe and Sanitary Bedding A. A home must include a designated area where each child can sit quietly or lie down to rest or nap. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Linens must be washed at least once a week and more often if soiled or dirty. All bedding and linens must be thoroughly cleaned and sanitized at least once a week and before use by another child. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens

    During the time of inspection, the Licensing Specialist observed, a can of bug spray that did not have an authorization form from the parent. Three diaper creams, one orajel, chest rub and two bottles of lotion that were in use that did not have an authorization form from the parent. CCF Handbook:7.24 Medication The family day care home or large family childcare home is not required to give medication; however, if they choose to do so, the following shall apply: A. The operator must have written authorization from the custodial parent or legal guardian to dispense prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and must contain the childs name; the name of the medication to be dispensed; and the date, time and amount of dosage to be given. This record shall be initialed or signed by the childcare personnel who gave the medication. Prescription and non-prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The childcare personnel must never administer a medication that is prescribed for one child to another child. L. Use of diaper creams and insect repellant may only be utilized with written permission from parents/guardians. Manufacturer instructions should be followed
