Complainant alleges that On 08/07/24, RS was picked up from daycare by the mother, and RS had a bruise/scratch on his face. The mother asked the teacher what happened, and all the teacher stated was that RS had a fall. No one notified RS's parent's when the incident occurred. Later on, that day, RS was crying and in tears and would not let the parents touch his arm. The school was contacted again by the parents, and finally the teacher stated that RS had fallen from a 4-foot obstacle on the playground. The parents took RS to urgent care and the x-ray showed that Reagan had a fracture to his arm. Statements were received from the teacher(CR), front desk staff(AS) and director(CD). Per the teacher, Ms. CR, she was on the playground caring for 11 children when the incident occurred. The teacher was looking the other direction at another child that was climbing a structure when RS fell. She heard him fall so she turned around and saw him on the floor. She picked him up, checked him and walked over to the child she was watching initially to prevent that child from falling. Ms. AS stated that she happened to go to the playground when the incident had just happened. She saw the child sitting with Ms. CR and Ms. CR said that the child fell. She checked the child but did not see any marks or bruising on his body. She proceeded to take him to the office for the Director to check him. The director checked him and completed an incident report. The child was later taken back to the playground. Per all staff members, the child didn't cry at all after the fall and he was moving his hands and arms without a problem. Video footage of incident observed and the child was observed falling off the second step of the playing structure. He fell face forward and did appear to cry or complain. There is an existing incident report but per director and office staff member, AS, they failed to give the mom a copy of the incident report at the end of the day.
