Complaint alleges that on 11/08/2025 at approximately 8:30 am, The teachers taped a child to a chair. The teachers taped his arms, legs and torso to the chair. They did this because they wanted EJ to learn to be still. EJ is a busy child. The teachers took pictures of EJ being taped to the chair. The teachers laughed at EJ being taped to the chair. The director and curriculum director were made aware and did not report the incident. EJ's parents were not notified. There are concerns about the teachers still being allowed to be in the same classroom as EJ. FINDINGS: A complaint inspection was conducted on 11/17/2025 and statements were received from the director, regional director, and teachers. Per the Director (E. Garcia), she was notified about the alleged incident on Friday, 11/14/2025, by Ms. A. Zupicich (Regional Director). A picture of the child tapped to the chair was sent to the H.R. department. The date on the allegation is wrong, we do not know the exact date but it is believed to have happened some time in early November. Per the Regional Director (A. Zupicich), On Friday she was alerted by H.R. that there was concern with one of the schools and a picture that was sent to them. She went to the center that same day and the director was not aware of the incident. She spoke to the teacher D. Rodriguez, through a Spanish translator employed by them. Ms. Rodriguez explained to her that the assistant director, Ms. Rasquin, went in the room and wrapped the child to the chair with tape. Ms. Rasquin thought it was funny and took a picture of the child. The child's teacher did not think it was funny and she told the assistant director to take the tape off immediately. Ms. Rodriguez took the tape off herself. Ms. Fernandez who was also in the room stated that she didn't see anything because she was changing a diaper at the time of the incident. Per the Teacher (D. Rodriguez), the incident occurred the morning of 11/7/2025. The assistant director went in the classroom with tape in her hands. She doesn't recall what EJ was doing at the time. The assistant director went to the child and taped him to the chair. She was laughing as she did it and took a picture of the child. Ms. Rodriguez told her to stop and proceeded to take the tape off the child. She doesn't believe the assistant director was being mean or malicious. She thought that she was playing with the child. Per second teacher (A. Fernandez), the assistant director came into the classroom with classroom supplies in her hand. She thought it was normal for her to go in the classroom because she is also an employee of the school and she works in the office. Ms. Fernandez was doing diaper/pullup changes and potty training in the restroom that is located inside the classroom. She didn't see or hear anything because her attention was on the children that were with her inside the restroom. She found out about the incident on Friday, 11/14/2025 when corporate visited the school. Per the assistant director (D. Rasquin), Ms. D. Rodriguez called the office and asked her to go to the classroom, thinking she was speaking to N. Barboza. She went to the classroom and the teachers said to her, 'Oh, I thought it was Narelis." Then, Ms. Fernandez proceeded to show her a picture of the child taped to the chair and refused to take it off because they wanted Ms. Barboza to see it first. The assistant director took a picture of the child and went back to show the director (E. Garcia) and assistant director(N. Barboza). N. Barboza kind of chuckled and the director was a little upset about the picture. The director said tell them to take it off, so she returned to the class to find him still taped to the chair. She took the tape off with Ms. Fernandez's help. Due to inconsistent statements received, It is unknown who of the 3 individuals taped the child to the chair. This complaint is substantiated and a non-compliance is being issued

A complaint allegation was received on 12/1/2023 alleging that On November 7th, parent received a call from the Director at TLE Sunrise stating that the child has blood present in her urine while she was on the potty. When parent followed up the day after with her teachers, the teacher that saw the blood wasn't there to talk to the parent and they didn't take any pictures of any sort to show the parents. There is inconsistency in their story . The child was taken to the ER on 11/07/2023 to address this matter. But the ER saw not one inch of blood or sign of infection. On another occasion the child said she was poked in the eye several times by another child but yet no incident report was made. A complaint inspection was conducted on 12/5/2023 and statements from the Teacher( Ms. C) and Director (CG) was received. Per teacher, she changed the child's diaper on date in question. She observed the blood in the child's diaper and informed the director (CG) and wrote an incident report. Per Director, the child has not had many incidents since she started child care. An accident/incident report related to the blood in the stool was not given to parent because they do not issue accident/incident reports for situations like this where there was no accident that happened at the school. An accident/incident report was not given to the parent relating to the poking of the eye because they are unaware of such incident. An accident/incident report was not given to licensing for review. This complaint is Substantiated as an accident/incident report nor any other document was provided to the parent documenting what the teacher observed while the child was in care

A complaint was received on 4/17/2023 alleging that a Classroom had way too many kids. when staff was asked where the other teacher was staff say all the staff comes in at 9. There were like 9 one year old's with one teacher. A ratio complaint inspection was conducted on 4/18/2023 and the following ratios were observed: infants 2:8, 1y/o 2:6, 1y/o 2:11, 2y/o 1:11, 3y/o 2:21, 4y/o 1:9, 4y/o and 5y/o 1:8, 4y/o and 5y/o 1:12, mixed group of ones to five y/o 2:18 (OUT OF RATIO) with at least one 1y/o. See supplemental for info on 1y/o. An attendance record was not assigned to the mixed out of ratio room as the children were assigned to different classes and were having breakfast in this room because parents paid for this event. Attendance for toddler A and B classrooms were checked for compliance and per the attendance, there were a total of 16 toddlers marked present prior to 8:30AM, of the 16 children, 15 were in toddler room B with two teachers (AM and BO). Per teachers, All the children from toddler A and B are received in the toddler B room, Until the third teacher(Y) comes in at 8:30AM and takes 6 children with her to the toddler A room. Between 8:00AM and 8:30AM, toddler B room is out out ratio based on the attendance records and staff on-site. Per the director, the front desk person, N, is her extra person in the mornings that assists with ratios but today she was not available as she was in another room assisting with breakfast. This complaint is Substantiated as the mixed ages room was observed out of ratio and per toddler attendance and staff schedule, the toddler room was also out of ratio

complaint description: CCLE Staff received a complaint on 2/28/23. The facility self reported a one year old child M.P. was left in the classroom as the teachers A.S and N.T. transitioned children to the playground. The center director C.G. was interviewed. Center director confirmed that she found child M.P. unattended in the one year old classroom. Center Director stated that the child M.P. was left unattended for approximately 1-2 minutes. Center Director stated there were 12 children in attendance and teachers A.S and N.T. did not follow attendance taking protocol when arriving on playing resulting in their failure to realize child M.P. was left behind. Teachers A.S. and N.T. were put on administrative leave pending the investigation. The parents of child M.P. were contacted and made aware of the situation. Teachers have returned to duty as of 3/1/23. Center Director has developed an action plan that involved retraining teachers and conducting classroom observations for the next 30 days. Complaint Outcome: CCLE staff has determined that this complaint is found to be Substantiated. A Notice of Violation will be issued. Date of Violation 3/01/23. Date Violation was issued 3/01/23
