The school self reported the following incident on 4/15/2026; A teacher restrained, pushed and struck M.V. with a book. FINDINGS: An investigation was executed on 4/16/2026, during which supporting documentation and witness statements were collected and analyzed. The accident/Incident report was reviewed. According to the report, a teacher notified the front office of an incident that had occurred during a reading activity. While another teacher (Ms. K.S.) was reading a book to the class, child (M.V.) began throwing toys at the teacher. The teacher was observed striking M.V. with a book. Ms. A.S. then decided to take M.V. for a walk. Per the Incident report timeline, the childs mother was notified of the incident at 7:50AM on 4/15/2026. The police, and CCLE were also notified on 4/15/2026. Per the owner, mother was notified the day after the incident because the allegation of what had transpired was brought to their attention near closing time. On the evening of 4/14/2026, The director initated communications with the regional director and the owner. They conducted their own investigation that night which included reviewing footage, if any, documenting the incident, terminating the staff member once the incident was verified, and communicating the information gathered to the abuse hotline. Per the teachers statement, she was in the room when the teacher struck the child. She didnt see everything that happened. She did see the teacher strike the child in her peripheral view. She didnt react quickly because she didnt know what to do at the moment. She eventually offered him a blue blanket that he likes to hold, and took him outside the classroom. Once they were outside, she offered him a hug and he refused. He didnt want to be touched so she told him she would just stay there with him. They returned to the classroom shortly after. She saw the assistant director walking the hallway, so she called her and told her that she needed to speak to the director. Once she was relieved of her duties, she told the director what she had observed. Video footage of incident collected. See complaint form for details. Complaint alleges that the daycare did not contact anyone during the day after they occurred and only gave a partial explanation for some of the injuries after the school day was over. No incident report was created. The daycare suggested that Sophia might have given herself the injuries or other children at the facility caused them. FINDINGS: A complaint inspection was conducted on 6/10/2025 and statements from the assistant director and the teacher was received. Per the assistant director, the office staff are assigned to writing incident reports. The teachers call the office to inform them of the incident when they happen and the office staff writes the report to give to the parents at the end of the day. The they did not see the parent when she picked up the child at the end of the day so the report was not given. The parent or child did not return after the day of the incident. Per the teacher, the teacher stated that she does not remember the child having any incidents where she could have received the scratches. She noticed the scratches when she got up from naptime. She noticed the marks on her back in the afternoon, at playground time. When the child gets upset, she cries and may self harms so she has to hold her so that she doesn't harm herself. On the day in question, she didn't have to hold her to calm her down because she didn't cry. She didn't see when/where the child could have gotten the bite mark or the scratches on her face. She noticed the bite mark under her shirt because she was playing with the child while at the playground. At one point, the child's shirt went up and that is when she saw the marks on he back. At pickup time, she informed the mom that she noticed the scratches on her face and the bite mark on her back. This complaint is substantiated as the incident report was not provided to the parent on the date of occurrence

The thermometer in the infant room was reading 54 degrees Fahrenheit during the walk-through. The monitor went back to the room to recheck the thermometer over an hour later and it was reading 50 degrees Fahrenheit which is above the appropriate allowable temperature. The director was advised to provide a new thermometer for the refrigerator to verify the temperature. She was also advised that if after changing the thermometer the reading was above the acceptable temperature, the refrigerator may need to be replaced

A complaint was received alleging a staff member (M. R.), scolded and yelled at a child. An investigation was conducted and a staff member (A. G.) who partially witnessed the incident was interviewed. A. G. stated that M. R. was heard yelling in the hallway, and exited the facility. M. R. was not available to be interviewed, however she did admit to the owner that she raised her voice when telling the child "we do not hit girls". This item is out of compliance as the staff member's behavior is not in accordance with the facility's discipline policy. THIS COMPLAINT IS DEEMED SUBSTANTIATED
