3.1 General Health and Safety Requirements B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. This includes grease build-up in ovens, stoves and food equipment. The bathroom exhaust fan vents were full of dust and lint. The director had the maintenance worker to clean vents before I completed the inspection, I observed the vents before departure

3.1 General Health and Safety Requirements I. All accessible electrical outlets must be tamper-resistant electrical outlets that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. In settings that do not have tamper-resistant electrical outlets, outlets shall have safety covers. There were five electrical outlet covers missing. All of the outlet covers were replaced before I departed the facility, I observed the new plug covers in place

40-12 · Medication
6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child. There is a child taking medication and the form was not completely filed out by parents. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.
