04-04 · Technical assistance was given regarding outdoor supervision. Provider retrained teachers regarding cell policy, outdoor supervision and serving children with additional needs. During inspection, Provider emailed Licensing Specialist documentation of retraining's and disciplinary action that was taken by facility for teacher who was viewed not supervising children on video footage. In the afternoon on May 6,2025 Provider informed Licensing Specialist that teacher is no longer employed at the facility. Per CCF Handbook, Section 2.4.1, A

Licensing Specialist observed two teachers on facility's camera footage the day the accident occured on the playground. Teacher #1 was observed sitting down, using cell phone and not supervising children on playground. Teacher #2 was observed supervising the children on the opposite side of playground, Copy of video footage was provided to the office of childcare licensing by the provider.
