Complaint alleges that 12/01/2025 at approximately 5:30pm, Parent walked into the daycare center to see a young black lady she's never seen before sitting on the counter by the door on her phone as her daughter sat across the room on the floor staring at the wall all alone soaking in pee to the point that the diaper is falling apart. Parent was upset that the young lady was not properly supervising the child. FINDNGS: A walkthrough of the classroom was done and the following was observed; Three children were observed sitting on the carpet watching media content on a tablet and the teacher was leaning on the shelf, watching her phone. When the young lady saw me, she quickly put down her phone and stood up. Per the assistant director, the teacher may have been updating the parent app. The teacher's parent app was checked and none of the children's pages were updated since around 2:45PM and teacher was observed watching her phone at around 4PM . This complaint is partly substantiated for inadequate supervision

Complainant alleges that a 2 year old child had two "slashes" on his face when picked up and no-one know what happened. An accident/incident report was created on 4/16/2024 after the parent noticed the scratches in the child's face during pick-up time on 4/15/2024. The teachers were not aware of the scratch and did not document it while the child was in care. Per Director (KM), she spoke to the teachers and the teachers saw him scratching his face but didn't see a scratch. This complaint is substantiated. Per staff, Video footage reviewed and child was observed to have scratched himself after scratching his face around 11:00AM. After reviewing the video, the child was observed walking around during lunch and scratching his face. There were two teacher present and one was directing him to sit down at the table
