A. During a routine inspection conducted on 4/1/2026, provider self-reported that on 3/31/2026 a parent told her that she saw through the camera system a teacher roughly handling a child during naptime. B. An additional complaint was received on the morning of 4/1/2026 alleging that teachers are roughly handling children. A child (K.P) had gone home with visible marks, such as bruises, scratches and a black eye. FINDINGS: A. A site inspection was conducted on 4/1/2026 and a statement from the director was received. The director stated that she reviewed the footage after the parent brought it to her attention and it in fact occurred. She stated that when she confronted the teacher about the incident, the teacher apologized to her. The director proceeded to terminate the teacher, and she is no longer working at the facility. Video footage of incident (3/31/2026 at 11:40AM) was reviewed and the following was observed: A one-year-old child (A.T.) was crawling away multiple times out of her nap mat. The teacher picked up the child and quickly brought her back to her nap mat. After multiple attempts to keep her on her mat, the teacher was observed roughly handling the child and pushing the childs head down on the mat several times. The teacher is also seen pulling the child by one arm to lay her face down, and restricting movement of her head. The father of the child was notified over the phone and in person about the incident. An incident report was created, given and signed by the childs mother. B. Per the director, the child (K.P.) has had many incidents at the school and incident reports have been created and shared with the mother. - Copies of accident/incident reports were reviewed for compliance. A. This self-reported complaint is substantiated. B. This complaint is partly substantiated as a teacher did handle a child in a rough manner. The complaint regarding marks on the child is unsubstantiated. Following progressive enforcement procedures, a Notice of Violation is being issued as this standard was previously cited out of compliance on 7/8/2025

Provider self-reported on 6/27/2025 that on 6/26/2025 a parent told her that she saw the teacher push a shelf into her daughter through the camera system. FINDINGS: A site inspection was conducted on 7/8/2025 and a statement from the director was received. The director stated that she reviewed the footage after the parent brought it to her attention and it in fact occurred. She stated that the teacher said she didn't see the child on the other side of the furniture. Video footage of incident was reviewed, and the following was observed; The child was crying and pushing the classroom shelf. Then, the teacher is seen walking toward the furniture from the right side of the screen. She pushes the furniture toward the child; the furniture makes contact with the child and the child's body slides over. The teacher is seen leaning over the shelf, talking to the child. She then, moves the shelf back and leans down to speak to the child with her finger pointing toward the child's face. She proceeds to move a second cubby into its place, near the child. She again, pushes the first shelf back in place and the child pushes back. This self-reported complaint is substantiated

Complaint alleges that a teacher physically abused a one year old boy by cleaning the floor with his own body. FINDINGS: A complaint inspection was conducted on 3/6/2025 and a statement from the director was received. Per the director, the incident occurred on 2/21/2025. The parent of the child in question called her to notify her that the teacher wiped the floor with the child. The director then reviewed the footage from the classroom and observed the child spilling water on the floor with his sippy cut. The teacher approached him and took his water bottle away. As the child was standing up from a sitting position, the teacher grabs him, wipes the floor with his bottom and stands him up. The Director called the teacher into her office, showed her the video and terminated her employment at that time. The director then contacted the mom and informed her of the actions that were taken following the incident. This complaint is substantiated as the school's discipline policy was not followed by the teacher in this incident
