Complaint alleges that there is a male person that resides in the home and he is always there in the evening hours, and drives a white car. FINDINGS: Per operator, her cousin, named Israel, lives in the home and he has been living there for 3 months and she doesnt know how long he will be living there. I asked her if he drives a white car, to which she stated yes, and the that she doesnt know much about cars so she cant tell me what type of car he owns. She didnt know if it was an SUV. sedan or van. This complaint is substantiated

Upon arrival, the operator and a guest were observed sitting in the living room area of the home. The children were near the front window, playing with blocks on the table. A black dog was observed walking around the table. The operator opened the door and said let me put the dogs away. She removed the black dog and placed it in the room. After a few minutes, a child went to use the restroom located in the hallway. The child opened the door, and a tan dog with light spots came out of the restroom and roamed the living room until the operator came out of the room and placed the dog in the room. TA was provided on keeping the dogs separate from the children during the hours of operation

Complaint alleges that the home operator asked a mom to leave a child with another parent at the licensed home until she returned. During the course of a complaint inspection regarding unsafe transportation of a child, the following non-compliance was observed; - The transportation logs for 5/11/2026, 4/15/2026 and 4/22/2026 were not on file and the operator did not have one for such dates. On 5/11/2026. the children were transported to drop off a child at home. On 4/15/2026 and 4/22/2026, the operator asked a parent via text message to drop off the child to her at location A. This was verified by the operators phone number, which verifies that the text messages were sent from her phone. The parent dropped off the child at location A but there is no transportation log to document the return to the home. The operator denied asking a parent to drop off the child to her at location A on 4/15/26 and 4/22/26. She stated that they were on a field trip at Welleby park and the parent was late so she asked her to drop her off at the park. Licensing inspector asked for the Welleby park field trip transportation log but the operator did not have one

A complaint allegation regarding transportation of a 4y/o child was received alleging that the child was placed in a potentially unsafe situation as she was seen riding in the front seat of the car without a booster seat. FINDINGS: A complaint inspection was conducted on 6/8/2026 and a statement from the home operator was received. The childs file was checked and her DOB is 5/22/2022. Per the operator, the child (N.V. 5/22/22) was riding in the front passenger side, in a booster seat. She stated yes, she was in the front seat, but she was in a booster seat. The incident occurred on 5/11/2026, so the child was 3 at the time of the incident. The operator showed me the booster seat that was used to transport the child and the front seat of the vehicle. A photo of both were taken. This complaint is substantiated, as the child must be seated in a back seat in an individual factory installed seat belt or federally approved child safety restraint. A notice of violation is being issued
