41-02 · Immunization Records
While reviewing children's files, the licensing specialist observed three student files (GKP, KR, CC) that had expired immunizations. The Child Care Facility Handbook states: 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health

23-05 · Technical assistance was provided regarding the servicing/tagging of fire extinguishers. Per the CCF Handbook, Section 3.8.2, B.... Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times (p. 27)
Counselor observed all fire extinguishers tagged for March 2022, signifying they needed to have been serviced by March 2023. At time of inspection provider scheduled to have the fire extinguishers serviced and tagged. Provider is to submit documentation of fire extinguishers being serviced by the due date

44-01 · Technical assistance was provided regarding maintaining personnel records. Per the CCF Handbook, Section 7.4 Personnel Records... Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. D. Copies of training information and credentials as applicable (pg. 61 - 62)

A screening conducted under this rule is valid for five years, at which time a re-screen must be conducted in the same manner as the initial screening. C. Child care personnel must be re-screened following a break in employment in the child care industry that exceeds 90 days. Child care personnel/individual with a break in service that exceeds 90 days are considered unscreened child care personnel/individuals until completion of re-screening. These child care personnel/individuals shall not have unsupervised contact with children in care
