39-03 · Accident/ Incident Notification and Documentation
A 2-year-old child left facility with a limp on 4.22.25. It was discovered during investigation that a virus had caused child to limp and have swelling on knee. Child was uncomfortable and not bearing weight at times on her leg between 4.22.24 and 4.24.25. Incidents reports were dated 4.22.25, 4.23.25 and 4.24.25 but were not generated until 4.28.25 and signed by parent on 4.30.25. 6.4 Accident/Incident Notification page 55 and 56 A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. E. In the event of serious injury or death, the incident must immediately be reported to the licensing authority

On 9/25/24 at approximately 4:30pm, a 2-year-old child was left unattended in outside play space by 2 staff. This was self-reported by facility. Both staff received written warning and retraining. Facility Administration is actively auditing the transition tracking each day to make sure name to face is appropriately completed and documented. 2 General Requirements page 12 and 13 2.4 Supervision 2.4.1 General Supervision Requirements B. On 9/23/24, Specialist observed video dated 9/20/24 of aggressive and hurtful behavior from one child to another. 2 staff that were responsible were engaged in an activity that did not focus on children's actions or behaviors in classroom. Staff members involved were reprimanded and given written warnings. 2 staff members to not complete the required training within time frame of start in industry. 4.1 Beginning Training page 39 Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the childcare industry in any licensed Florida child care facility. 129 children enrolled 25 immunization records out of compliance. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. A. Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida.
