    During the inspection, Licensing Specialist observed the substitute did not complete their School Readiness training within the 90 days. The provider was given until May 8, 2026, to come into compliance with this standard. During the inspection, Licensing Counselor provided technical assistance, School Readiness Program Health and Safety Standards Handbook Section 3.1 Preservice. Child care personnel must successfully complete the preservice training coursework described in this section, as applicable. All child care personnel must complete these preservice training requirements within 90 days of initial employment with any School Readiness provider. This timeframe does not start over if personnel change employment to another School Readiness provider within this initial 90 days. Personnel who have not completed all preservice training requirements are not allowed any unsupervised contact with or care of children in a School Readiness program

    During the time of inspection, Licensing Specialist observed on the playground the ground cover is accessible and shredding. The pavilion the paint is peeling. The plastic caterpillar toy tail was broken and has a sharp piece. The fabric hanging swing is ripped and has grease on it. the step 2 jungle gym the window is cracked and there is a black substance on it. There is a black and green substance on the fence. The bottom bar of the climbing dome was bent inwards. The plastic covering on the wooden jungle gym is cracked and peeling off. The bottom step on the wooden jungle gym is broken, at the top there the wood is deteriorating and there is a sharp piece. The plastic on the wooden gym has a black substance on it. The see saw the handle bar is rusted and have sharp pieces. The director was given until May 8, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook 7.6 Outdoor Time, Fencing and Play Area Requirements A.     During the inspection, the licensing specialist observed there was Nystatin and Boudreauxs Butt Paste diaper ointment sitting on the diaper changing table. The children had access to the diaper ointment. The director brought this standard back in compliance at the time of inspection by removing the diaper ointments and placing it in an locked cabinet. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 7.24, H Medication H. All medicine must have child resistant caps, if applicable, and shall either be stored in a locked area or must be inaccessible and out of a childs reach

During the inspection, Licensing Specialist provided technical assistance, The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations

Staff person showed Licensing Specialist where two other bolts were cut back as well. Licensing Specialist observed that the large fort style climber continues to have some corrosion in one of the wooden legs, some corrosion on the window board facing the back fence, and a board sticking out at the ground level with two exposed screws, presenting a pinch concern. "7.6, A. Outdoor Play areas shall be clean, in good repair and free from litter, nails, glass, and other hazards." The provider will repair, replace, or remove the large fort climber no later than March 16, 2024. Licensing Specialist will return after this date to observe for compliance
