During this inspection, Counselor observed the flooring in one (1) year old; two (2) year old and three (3) year old playground has an uneven surface. The artificial turn in all of the playgrounds have depressions in the flooring which may result in children tripping/falling. The playground surfaces have what may be roots underneath the turn which a child could stumble and fall. The playground for the one- year-olds has weeds growing in the area (picture) and there is a black-greenish substance on the fence within reach of the children. The two-year old playground has a Toy, Finger Paint sign, has broken plastic (picture); there is also a large screen protruding out of the ground. All of the playground surfaces, with the exception of the one-year-old playground, are covered with dried/dead leaves which are accessible to children. (pictures taken)
