Specialist observed that one teacher had not started the introductory training within 90 days of beginning in the industry. The Child Care Facility Handbook states on page 39: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. Specialist observed that one staff member had not completed safe sleep within 30 days of hire. The Child Care Facility Handbook states on page 41: 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. The Child Care Facility Handbook states on page 60: Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. 04-01 · Supervision
A 3 year old child at this facility, who was not potty trained, was taken to the infant room to be changed. The child was put on the changing table used for the younger children and fell off of the table, breaking her collar bone. This high changing table should not be used for older children and the changing mat should fit securely on the table with the child securely strapped. The CCF Handbook states that child care personnel must position themselves so that all children can be directly supervised

A 3 year old child fell off of a changing table on 4/28/2023. The staff member failed to notify administration or the parents of the incident. On 4/30/2023, the child's parent contacted the program director to notify her that her child had a broken collar bone and it happened at the child care center. The program director attempted to get a signature on an incident report on 5/1/2023. The parent refused to sign the report. The CCF Handbook states that documentation must be shared with the custodial parent or legal guardian on the date of occurrence
