The self-report stating that the teachers did a name-to-face count outside but did not count the children again once they got inside leaving one child outside alone has been confirmed. The center has already made changes to try to prevent such an incident from happening in the future. They are requiring more name-to- face counts and require the 2s classes to use walking ropes during transitions

The incident report wasnt written and signed on the day the incident occurred. That was done on the next morning. The licensing specialist gave a reminder that accident/incident reports are required to be written and signed on the day the incident occurs. Please submit a corrective action plan that will ensure that all staff are aware that they must document accidents/incidents on the day of occurrence
