34-04 · Credentialed Staff
The Licensing Specialist did not visit the facility as this is a paper inspection. The Director listed at the facility has an expired Director Credential that expired on 3/5/25. The facility will be placed on a 6 month provisional license. Per the DCF Handbook Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. During the time of the inspection the Licensing Specialist observed that the annual vehicle inspection was not valid as it expired on 9/27/24. During the time of the inspection the Licensing Specialist observed that each classroom had paint chipping on the yellow and blue walls and the doors/walls had dirt build up throughout the facility. Per the DCF Handbook A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

During the time of the inspection the Licensing Specialist observed the diaper changing table next to the bottle prep area. This was corrected at the time of the inspection as the Director moved the diaper changing table and put all the bottle prep items in the kitchen to be prepared. During the time of the inspection the Licensing Specialist observed that the fall zones under the swings and 2 metal climbing gyms had less then 1 inch of mulch under all of them. Per the DCF Handbook Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls

During the time of the inspection the Child Care Licensing Specialist reviewed the driver's personal file in which the annual physical was expired. Per the DCF Handbook E. For the driver only: Florida drivers license and driver physical examination documentation. A copy of the drivers license and the physician certification, or another form containing the same elements of the physician certification, granting medical approval to operate the vehicle, and valid certificate(s) of course completion for first aid training, pediatric cardiopulmonary resuscitation (CPR) procedures, and background screening must also be maintained in the drivers personnel file

During the time of the inspection the Child Care Licensing specialist observed 1 bottle and 1 sippy cup in the Infant room not labeled with a child's first and last name. This was corrected at the time of the inspection as the teacher in the Infant room labeled both items with the child's first and last name. Per the DCF handbook Section 3.9.5, B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily

During the time of the inspection The Child Care Specialist reviewed 2 children's files missing a Student Health Examination/DH (Form 3040). Per the DCF Handbook 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination
