At the time of the complaint, the Specialist learned that the provider notified the Department that an incident occurred at the facility; however, failed to provide accurate details. The information relayed was that the child had chairs accidentally fall onto him and no injuries occurred as a result. At the time of the commencement of the inspection, information obtained through interviews acknowledged a child was injured as a result of teacher accidentally dropping a chair; scratching his face. The admin stated specifically that the child had a mark on their face that she questioned the teachers about when she was shown the child's injury wondering how it occurred. A video of the classroom's surveillance had to be watched to explain what occurred. LE who responded to the parent's call observed a small mark on the childs eye and also mentioned the teacher's lack of response to the child's crying reaction in the video. Accurate information to the department was not reported; the child DID sustain an injury from the incident, which resulted in the teacher being terminated by the facility because the teacher did not show consolation or aid to the child's crying reaction and injury left on child's face. It is advised that when reporting, accurate and all info is documented to ensure that reports are screened in/out based on specific details
