4-01: During the inspection, Licensing Specialist observed that the children in the four year old/five year old classroom was inadequately supervised, in that there was one teacher in classroom that left the children unsupervised when she left the classroom to answer the front door. 46-3:During the inspection, Licensing Specialist observed that the infant and the three year old classroom did not have an accurate attendance. The director stated that the facility uses Brightwheel to track attendances and that the teachers have access to Brightwheel on the tablet or on their cell phones. The director stated that at around 9:00 am, staff will use a paper attendance as well. During the inspection, the teachers in the infant classroom and three year old classroom could not log into Brightwheel and the paper attendance was blank. During the inspection, Licensing Specialist observed paint peeling in the infant room on the wall by the fire alarm (right side of room). In the infant room a hole at the bottom of the 1st entrance door on the inside left side of the door there are sharp edges. Outside of the two year old's room at the bottom left side of the door there is a hole that has nails sticking out and sharp pieces. During the inspection, Licensing Specialist observed on the playground the base of the fence had a gap that measure 5 inches. The gap was close to the road on the right side of gate. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. • During the inspection, Licensing Specialist observed in the freezer in the kitchen was strawberries stored in an open zip lock bag. The licensing specialist also observed left over food from breakfast (cream of wheat and apples) that morning in bowls that did not have seal that was tight and was not labeled with the date on it. The staff threw the strawberries into the garbage. The staff labeled the food and put saran wrap that was tight fitting over the food.
