    At the time of the inspection observed the chain link fence on the outdoor play area with a height less than 4 feet. The fence located on the North side of the out door play area measured approximately 3ft. 6inches. The fence located on the East side of the outdoor play area measured between 3ft. 9 inches and 3ft. 10 1/2inches. TA-- The outdoor play space shall be fenced, a minimum of four (4) feet in height. At the time of the inspection, the Operator spoke with the Environmental Manager (C.D) to request an extension of 30 days to repair the fence. The extension was requested due to financial issues and the time constraint. The extension was granted and the Operator was informed that compliance was required in 30 days for renewal of the annual license. She was also informed that if compliance was not met on the reinspection date, she will be required to attend the Child Care Advisory Council to seek approval for a provisional license. On 4/28/2025, the Operator informed the DOH that the she was unable to repair the fence in the outdoor play area by the reinspection date of 4/28/2025 due to permitting issues. The Operator requested an extension verbally and via e-mail. Environmental Manager (C.D.) approved a 30 day extension for repair of the fence (5/28/2025) The Operator was once again informed that she will be required to attend the Child Care Advisory Council Meeting to seek approval for a provisional license. The next CCAC meeting will be held on 5/14/2025. See Supplemental Page for extension request sent via e-mail

    Observed the outside area, behind the unlocked fence there are items such as, rusty container of water seal/ spray container of spray paint located in yellow bin. Also There is Large Storage unit located on the play area, The door to storage unit was left unlocked which gave children access to a bag of Triazicide Insect killer/ spray can of Wasp and Cornet killer spray, Bottle of Round up . Inside red container there is multiple bottles such as lighter fluid and bottles of fluids that use to put inside a vehicle. The provider made corrections by locking the gate/ storage area
