3-14 [SR] Class 3 Employment History Missing [SR] Violation Detail A five-year employment history verification was not completed and on file for personnel prior to hire. (Section 18.4, number 2) At the time of the LFCCH License Renewal Inspection, the provider and staff members files were missing the 5-year employment history check as well as the 3 reference letters. Technical Assistance was given to the provider by explaining to them that a 5-year employment history check with 3 reference letters need to be kept in all staff files
