12-19 · Facility Environment
During the time of the inspection, the Licensing Specialist reviewed the camera footage from the playground area on the day of the incident. The Licensing Specialist observed that the staff did not follow the facilities written protocol (date started unknown) of Before taking the children outside to play, do the following, conduct an outdoor surface temperature check before outdoor play. The Licensing Specialist observed that the teachers and children both went into the playground area at the same time, with some of the children running ahead of the teachers. The playground area and temperature check were not completed as thoroughly as the written paper protocol is outlined. According to the childcare facility handbook 3 Physical Environment Children are much more susceptible to the adverse effects of environmental factors, materials and toxicants. It is imperative that the childcare environment is conducive to the healthy development of children. 3.1 General Health and Safety Requirements A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

17-10 · Outdoor Play Area/Fencing
During the time of the inspection, the Licensing Specialist observed that the playground equipment on which the child was allegedly burned has a partial unshaded area. The treehouse's plastic sliding board portion was not positioned in the shaded area. The sliding board was the only part of the play area not covered by the tent for shade. The excessive sun caused the sliding board to be extremely hot therefore causing injury to the child's hands. According to the childcare facility handbook 3.5 Outdoor Play Area D. Children must not come into contact with any surface or equipment which poses a burn risk

39-04 · Accident/ Incident Notification and Documentation
During the time of the inspection, the Licensing Specialist was advised that the parent did not receive the incident report on the day of the incident. The child was picked up from the facility shortly after the incident occurred by the parent. However, the parent did not receive the incident but report. The child is no longer enrolled at the center. According to the childcare facility handbook 6.4 Accident/Incident Notification B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. D. The documentation must be maintained for 12 months. During the time of the inspection, the childcare personnel advised the Licensing Specialist that she placed diaper cream on the child's hand after the child sustained burn injury to her hands. 33-01 · Training Requirements
During the time of the inspection, the Licensing Specialist observed that one childcare personnel did not complete their 40-hour introductory training within a year of hire. 4.2 Training Requirements According to the childcare facility handbook 4.2.1 Mandated Introductory Training Childcare personnel must successfully complete 40 hours of childcare training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Childcare personnel who successfully completed the mandatory 40-hour Introductory Childcare Training prior to January 1, 2004, are not required to fulfill the competency examination requirement. A. According to the childcare facility handbook 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department.
