During the time inspection, Licensing noticed two incident reports. Each incident reports were insufficient, they were missing the time the incident occurred. One of the reports was missing the parents' signature. The other report was missing the staff that were present the day off the incident. According to the DCF Handbook Section 6.4 Accident/Incident Notification C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program childcare personnel and the custodial parent or legal guardian. During the time of the complaint inspection the Licensing Specialist obsereved that the daily attendance on the day of the incident was not accurate with the arrival and departure times of the childcare personnel. During the time of the inspection, the Licensing Specialist measured the mulch around the fall zone and did not measure the required 6 inches of depth. During the time of the inspection, the Licensing Specialist provided technical assistance. CCF Handbook, section 3.12 Outdoor Equipment D. 1. During the time of the inspection, the Director advised the Licensing Specialist that there are currently 25 employees. During the time of the inspection, the Licensing Specialist monitored 25 of the 25 employee records. During the time of the inspection, the Licensing Specialist observed that multiple employees have not completed their 40 hours Introductory Child Care Training Requirements. During the time of the inspection, the Licensing Specialist provided technical assistance. CCF Handbook, section 4.2 Training Requirements Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement
