During the time of inspection, the Licensing Specialist observed 15 of the 20 employee files did not have current Child Abuse & Neglect form (CF-FSP 5337). The Director was able to obtain signatures of all staff at the time of inspection. This standard was brought back into compliance. CCF Handbook Section 7.4, C Personnel Records Records must be maintained and kept current on all childcare personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before Child Care Facility Handbook Page | 62 hire date and annually thereafter by all childcare personnel. D. Copies of training information and credentials as applicable. E. For the driver only: Florida drivers license and driver physical examination documentation. A copy of the drivers license and the physician certification, or another form containing the same elements of the physician certification, granting medical approval to operate the vehicle, and valid certificate(s) of course completion for first aid training, pediatric cardiopulmonary resuscitation (CPR) procedures, and background screening must also be maintained in the drivers personnel file. F. Prior to beginning volunteering in a childcare facility, a CF-FSP 5217, Volunteer Acknowledgment which is incorporated by reference in 65C-22.001(7)(e), F.A.C., and may be obtained from the Departments website www.myflfamilies.com/childcare, must be completed and on file at the childcare facility for the volunteer. Written documentation of volunteer hours must be maintained at the facility for a minimum of 12 months and available for review by the licensing authority

During the time of inspection, the Licensing Specialist observed ten children files did not have updated immunization records on file. Per DCF Handbook Section 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. F. It is recommended that childcare personnel are current with all immunizations routinely recommended for adults by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC), as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by in 65C-22.001(7)(v), F.A.C
