During the time of the compliant licensing was advise that the incident report was documented after the child and parent had left the facility. According to the DCF Handbook Section 6.4. 06-02 · Driver's License, Physician Certification
During the time of the inspection the Licensing Specialist observed that the driver's physical was expired. This was resolved at the time of the inspection. According to DCF Handbook Section 2.5.1 Driver Requirements: B. An annual physical examination which grants medical approval to drive, and valid certificate(s) of course completion for first aid training and pediatric cardiopulmonary resuscitation (CPR) procedures. Licensing specialist observed the driver had driven after her physical was expired by looking at van logs with her signature between the dates of 10/09/23 - 11/19/24. During the time of the inspection, the Director advised that there were 183 children enrolled. During the time of the inspection, the Licensing Specialist monitored 183 of the 183 children's records. Licensing Specialist found one child who has a missing physical form. The director was given until January 19, 2024, to come into compliance with this standard. During the time of the inspection, the licensing specialist provided technical assistance. CCF handbook ,7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized Child Care Facility Handbook Page | 60 professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. 15-03 · Technical assistance was provided: One teacher took her class ( 11 children) to the cafeteria to take there snack. Thus returning this standard into compliance at the time of this inspection
CCRC observed 29 children in the three and Four-Year-old-class, class capacity was 23. Director informed CCRC that they merged the classes as the other class was used by a specialist that was working with a child

39-05 · Accident/ Incident Notification and Documentation
CCRC observed three incident reports alleges with this complaint. CCRC asked to see the footage for these incidents. Director informed CCRC that she looked for them and she was not able to find them according to time listed on the reports. All incidents happened during naptime and the time on incident is not correct as the child was not in the facility at that time. Technical Assistance was provided: CCRC explained requirements to the Director. Director held a meeting with all teachers and provided training on Incident/accident reports requirements. Director sent training form signed by all teachers that they had attended the meeting via a way of text. Name of form was How to Fill Out a Boo Boo Report. Thus returning this standard into compliance at this time. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. E. In the event of serious injury or death, the incident must immediately be reported to the licensing authority
