3 Physical Environment 3.1 General Health and Safety Requirements E. Pursuant to Chapter 386.204, F.S., smoking is prohibited within the child care facility and in vehicles when being used to transport children. Smoking is prohibited in all outdoor areas, including on field trips, while children are in care. Owners/operators are to notify custodial parents and legal guardians, in writing, that smoking, including ecig We observed a staff member smoking on the side steps of the facility, I advised the director that the staff member was observed smoking outside the building. 3.8.4 Fire Drills B. When the facilitys approved fire alarm alert system is activated or initiated, all adults and children must evacuate the facility. We observed the facility fire alarm was unable to sound, once director and other staff members tried to activate the system. Fire drill was still conducted by director shouting fire drill. Director was advised to immediately notify the alarm company

03-01 · Ratio Sufficient
Ratio: LC observed the entire facility to be out of ratio except the 1 year old classroom located in the back right. The preschool teacher had 20 three and four year old's. Majority three year old's. The 1 year old teacher located in the back left had twelve 1 year old's and in the infant room, there was teacher and other personnel watching 8 infants. Incorrect ratio: 1:20 - preschoolers (majority 3 year old's) 1:13 - 1 year old's 1:6 - 1 year old's 2:8 - Infants (other personnel caring for infants) Corrective action: The owner called parents to come pick up 5 children from the preschool classroom and 4 infants from the infant room. Another staff member that arrived went into the 1 year old classroom. Corrected ratio: 1:15 - preschoolers 2:13 - 1 year old's 1:6 - 1 year old's 1:4 - Infants 2 additional staff member present

LC observed a preschooler in a highchair. LC questioned the teacher, and she stated the child was hitting other children, so she put him in the highchair. LC informed the teacher that she cannot do that and as an alternative, to put the child in a corner and give him a book. Corrective action: The teacher removed the child from the highchair

Staff listed on the supplemental has not completed their required ELC training. TA: Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. One of the Departments approved literacy courses.
