At time of the inspection, the Licensing Specialist observed one staff records did not have documentation of five years of employment history in the file. The Licensing Specialist took pictures. The Licensing Specialist provided technical assistance from the handbook 5.1, D The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained

At the time of inspection, the counselor observed bottles of sanitizer wipes stored in the two bottom drawers of an unlocked file cabinet, in an unlocked closet in the classroom accessible to children. Pictures taken. The director moved the bottles and placed them on the high shelves in the closet bringing the center back into compliance immediately. Provided technical assistance from the handbook 3.2 B 3.2 Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times

At the time of the inspection, the counselor observed the right-hand fence near the gate area upon entering had a gap at the bottom so it was not at ground level. en out. The gap measured more than 3 and half inches. Pictures taken. Provided technical assistance from the handbook 3.5 F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided
