On March 20, 2025, a child snuck away from a teacher as the teacher was retrieving another child and was left on the playground hidden from view. The child knocked on a window of another classroom and the teacher escorted him to his classroom. One of the teachers in the 2-year-old classroom was terminated on 3/21/2025. Specialist interviewed the teacher that witnessed the incident February 20th, 2025. She was busy and forgot to notify the parent or have the parent sign the accident/incident report when the child was picked up. When the parent came to pick the child on the last day of enrollment, they asked for all accident/incident reports involving their child. That was when it was discovered that the parent was not notified. The parents signed the incident report once it was discovered. The Child Care Facility Handbook states on pages 55 and 56: B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form

A child had what appeared to be a medical emergency, as witnessed on video. The teachers did not report to the administration or parents. The two teachers were terminated. The Child Care Facility Handbook states on page 55: B. For life threatening injuries, the provider shall call 911 and then notify the parent. C. Custodial parents or legal guardians must be notified immediately if the event of any serious illness, accident, injury or emergency to their child, and their specific instructions regarding action to be taken under such circumstances must be obtained and followed.
