During this inspection it was noted that the monitor could not close many of the seatbelts on the vehicle. All seatbelts must be in working order with both parts available to ensure that the children are secured. The facility cannot place any child in a seat in which a seatbelt is not working. Repair the hole in the side of the drivers seat and repair the seat behind the driver (cushion and seat back) The facility will bring the vehicle to the child care licensing office when all concerns on the vehicle have been corrected

While reviewing the transportation log for the trips to Markham Elementary School, Charles Drew Elementary School and Cypress Elementary School it was noted that the time recorded for departure from these three schools was the same although they are three different stops. Times of departure need to be recorded as the vehicle leaves each elementary school. The facility was provided with technical assistance on how to complete a transportation log. Upon arrival at 2:35 to the facility a walk through was conducted and it was determined that there were 24 children on site. Of those 24 children present 2 were in an unlicensed space (temple). There were six children on the facility's vehicle which was verified by the facility's transportation log. This brings the total number of children in care to 30. The licensed capacity is 24 and they exceeded it by a total of 6 children. The facility had been cited for being over capacity during a complaint investigation on March 31, 2023

A complaint allegation was received on 3/30/2023 alleging that a child was inappropriately touched by another student and the staff member, Mr. Zantwon, was not watching the children because he was on his cell phone. A statement was received from the Director/owner, SM. Per Ms.SM's statement, Mr. Z only watches the children when the staff members have to use the restroom and he is the person who does the cleaning at the facility. She stated that the is aware of the allegation because the parent mentioned it but the staff did not see anything. This complaint is Unsubstantiated. A walkthrough of the facility revealed a lack of direct supervision in the temple/ hallway. 3 children observed alone, napping in the temple and 1 one-year-old child was observed in a play-yard near an exit door

A complaint allegation was received alleging that the preschool is at capacity and the children are being taken to another school. Per Director/owner, there were 22 children present during inspection. A complaint inspection was conducted on 3/31/2023 and a walkthrough of the facility revealed the following; 22 children inside the facility. I asked them to let me see the playground because I heard children. They told me to hold on, therefore, i proceeded to open the back door myself. At that point, I saw a group of children walking toward the left side of the building with 2 adults. They appeared to be preschool children, so I asked the teacher (Ms. Marsha) who the children were and she stated that they belong to the private school located in the corner of the strip. I felt like I was being lied to so I followed the group of children around the building. I observed that most of the children were wearing McCloud Christian Academy Shirts and the teacher that was with them was a staff member mentioned on the complaint allegation (Mr. Zantwom). I counted a total of 18 children in this group, bringing the total count to 40 children. The license capacity is 24 and they exceeded it by a total of 16 children. Of the 22 children inside, the building, 4 were observed in an unlicensed space (Temple/hallway adjoining temple). TA was provided on the following; - Seek to add the temple to the facility's licensed capacity - Tell the truth, It is a misdemeanor of the first degree to make misrepresentations, by act or omission, regarding the licensure or operation of a child care facility or family day care home to a parent who has a child placed in the facility or home or is inquiring as to placing a child in the facility or home, or to a representative of the local licensing agency, or to a representative of law enforcement, including but not limited to, any misrepresentation as to: the number of children at the child care facility, etc

I checked the 2y/o and 3y/o attendance and it did not match the number of children present. TA was provided that attendance must match the number of children occupying that space.
