A complaint allegation was received alleging that on November 13th or 17th of 2023 child (AD) fell and parents were not notified. The childs ankle was swollen and after speaking with the director, she said nothing had happened because the children didn't go outside. After parent requested to see video footage, they noticed that the child fell on the uneven pavement outside. The child was taken to hospital by the parent and no incident report by anyone at the school was given to the parents. Parent believes her child has been neglected. A complaint inspection was conducted on 1/23/2024 and statements from the teachers (RS and JI) were received. Per Ms. JI, the children did go outside because the incident occurred on the playground but she didn't see it happen. The child was in another area of the playground playing by himself. The teacher became aware that something happened after the mother expressed concerns about the child's ankle being swollen. The director showed her the video later and that is how she became aware that he had fallen on the playground. Per Ms. RSJ, she was in the classroom when the incident happened on the playground but she saw the video footage of the incident when the director showed it to them. This complaint is substantiated for inadequate supervision of the child on the playground. Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs

Provider self reported an incident on Monday 1/22/2024 that teacher AJ grabbed a student roughly by the arm and was terminated for it. A complaint allegation was received on 1/24/2024 alleging that last Friday, 01/19/24, there was an incident with the teacher at the child care center. On Monday, the family was told about the incident. On Friday, the teacher grabbed EH aggressively. The teacher was fired because of that incident. After seeing footage of what occurred, it was noted that the teacher picked EH up by the arm and then slung her with force onto the cot. Then the teacher was pushing her down. EH then got on her knees. The teacher smashed her down and pushed her head down with force. EH was thrown back onto the cot, and she hit her head. She was smashed in the face; the teacher took EH by the hand and made her hit herself. A complaint inspection was conducted on 1/23/2024 and two video footages of incident were also checked for compliance. Per the first video footage, the teacher was observed pulling the child up by the arm and putting her on the cot. The child got up again and teacher was seen pulling her by one arms, put her in the cot and made her slap herself on the arm. On the second video, teacher was observed leaving room to retrieve the child from the hallway. The teacher carried her by pulling her up by both arms and laying her down on the cot. Child sat up so teacher pulled her down by the shirt forcing her to lie down. She proceeded to pin her down with one hand while holding her phone to her ear with the other hand. Child continued to sit up and teacher tried forcing her down. At that point, another staff member observed walking into the class to speak to the teacher. This complaint is Substantiated and a class 1 non-compliance and Notice of Violation is being issued
