During the course of a complaint inspection, Video footage of alleged incident was requested and reviewed. The following was observed; Child A - Unknown child that caused the scratches Child B - The injured child Video stamp date is 3/17/2025 at 8:59AM. Three teachers were observed standing near the playground gate and the children are playing around the playground. The alleged incident is seen to have occurred behind the wooden play structure near the back of the playground. Child A is observed grabbing child B, by wrapping his arms from behind her. Child A drops his body back and both children fall back. At that point, both children are laying on the floor. Child B sits up and Child A throws himself on her, causing her to fall back to a laying position on the ground. At that point, children are seen gathering around, and one child goes to get a teacher. The teacher looks over and sees the commotion. At that point, she walks toward the children and tends to the child. The incident lasted about 40 seconds. A non-compliance for inadequate supervision is being issued. TA was provided regarding constant and direct supervision while on the playground

Complaint alleges that child (girl) had an incident at the school and an incident report nor a call was made to the parents on the day of the incident. School is refusing to show footage of incident to the parents. FINDINGS: A complaint inspection was initiated on 3/20/2025 and completed on 3/21/2025. Per the director, the incident happened on the playground on Monday, 3/17/2025. That morning, there were 3 teachers on the playground so there were enough staff to cover ratio until the rest of the teachers arrived at 9am. After the incident, there were several events that happened that kept her occupied and she could not call the parents. The accident report was issued in the morning on 3/18/2025. This complaint is substantiated as an accident /incident report was not issued on the date of occurrence. TA was provided on report documentation on date of occurrence
