CCLE was notified by another agency on 7/2/25, that on 4/14/25 an incident had occurred at that facility that read as followed: "There are concerns for the children under a specific teachers care because she has a history of leaving children in her care unsupervised because she forgets about them. In April 2025, the particular teacher forgot a child outside in the playground for around 30 minutes. When the parents arrived to pick her up, it was the parents who found her outside by herself. The mother was upset and reported the teacher to the director. The child was not harmed. The teacher has also forgotten other children in the bathroom and other places, but since other staff workers were around, the other children were quickly found." CCLE conducted a visit at the facility on 7/2/25 where the director admitted that on 4/14/2025 a two-year-old child was left alone on the playground between the hours of 4:17PM and 4:23PM and the parent found their child on the playground. See complaint form for details. This complaint is SUBSTANTIATED and a class 1 non-compliance and Notice of Violation is being issued

During the course of a complaint inspection regarding supervision, the following non-compliance was observed related to the complaint; On Monday, 4/14/2025, a child was left on the playground because she had been signed out by the parent ahead of time. The parent had not accessed the playground prior to signing the child out. The class was in transition to the classroom and they checked the Brightwheel app to see how many children were present. According to the attendance on Brightwheel, , Teacher A had two children in her care but there were 3 children present, as the parent had not yet retrieved the child. TA was provided that the childs arrival and departure must be recorded by child care personnel at the time the child enters and departs the child care facility

The fire extinguisher needs to be inspected annually by a fire equipment specialist. There was no documentation that the fire extinguisher had been inspected. Fire extinguisher for buses vin#63030, and 03529 tag dated 6/23. Bus vin #16869 has no service tag, provider explained that the bus was bought in 10/2023.
