Provider self-reported the following incident: On 5/28/2025, a child was left alone/without adult supervision in the vehicle during morning drop-off. Findings: An on-site investigation was conducted on 5/29/2025. A statement from the Owner was received and video footage of incident was reviewed. Per the Owner T.D., the school provides morning transportation from home to the school and afternoon transportation from school to home. On this specific incident, on 5/28/2025, the children were picked up in the morning and brough to the school at around 8AM. There are two drop-off locations (East side and West side). The East side is the school's transportation vehicle drop-off location and the West side is the parent's drop-off location (car loop). On 5/28/2025, the driver A.A. dropped off on the west side instead. The staff person assigned to receive children on the west side is not trained to unload the children from the vehicle but she felt pressured to unload them. After unloading them, the driver parked under a tree and went home early because she felt sick. About an hour later, another child's father heard a cry coming from the school's vehicle and alerted the school staff that a child was left in the school van. The staff opened the van and let the child out. The child's eyes were bloodshot red and he was hot and red from the heat. The owner took the child inside and placed ice packs under his arms, on his neck and back. Herself and other staff members provided water to the child. They called the driver and asked her to return to school because she had to speak to the parents. After tending to the child, they contacted the parents who came and had a conversation with the driver of the vehicle. Videos of incident were viewed and the following was observed: Video 1 At 7:59, Van B, is seen pulling into the parking lot. The driver got out of the vehicle and opened the passenger side sliding door. Approx. 9 children hop off the van and she closes the door behind them. The driver has a conversation with the admin. assistant and hands her an item. She then walks back around the vehicle and climbs on the driver seat. At 8:01AM, she pulled away and parked under a shaded parking spot in the parking lot. At 8:02AM, she walks out of the vehicle and toward the school entrance. From 8:02AM - 9:17AM, the child was in the van unnoticed until a couple walked by and approached the van after something appeared to catch their attention. The male goes to the school doors to notify the staff. At approx. 9:20AM, the staff come outside and learn that a child was left in the van. Video 2 Per the second video's date and time stamp on 5/28/2025 at 9:21AM, Teacher A.F. is seen running toward the van. One side of the van is in camera view and the rest of the van is blocked by a tree. The teacher is seen walking out of camera view toward the right side of the van out of camera view. Then, she is seen coming back into camera view with the child. The Owner is seen running toward the teacher and child. She takes the child from the teacher and leads the child inside the school. It was determined that the child was left in the van, without adult supervision, from 8:02AM until 9:21AM. A Class 1 non-compliance and Notice of Violation is being issued

During the course of a complaint inspection where a child was left unattended inside the vehicle on 5/28/2025, the following non- compliance related to the incident was confirmed; The alarm's deactivation mechanism was installed but "not activated" as per the alarm system announcement. In this instance, the driver is seen on video parking the vehicle and getting off it without deactivating the alarm. During my visit on 5/30/2025, the vehicle alarm was checked and it is not in working order. When the vehicle is turned off, the alarm states that it is not activated. I could not get further information from the owner about how long the children have been transported with an inactive alarm

During the course of a complaint inspection where a child was left unattended inside the vehicle on 5/28/2025, the following non- compliance related to the incident was confirmed; Per the school owner's statement and as evidenced by the transportation log and video of incident; The driver failed to conduct a physical inspection and visual sweep of the vehicle to ensure that no child was left in the vehicle. The second person also failed to conduct a sweep of the vehicle. The transportation logs were not signed and/or dated to verify that all children were accounted for. Due to the severity of this incident, the non-compliance is being escalated to a Class 1 Non-compliance and a Notice of Violation is being issued

During the course of a complaint inspection related to supervision, this non-compliance is being issued. The van driver denied to provide a statement to the licensing authority when asked to provide one. She stated that her lawyer had to be on a three-way call in order for her to provide a statement. When asked for the lawyer's information so that Child Care Licensing can arrange a time to speak, she refused to provide it. The school owner provided a statement on the initial visit, 5/29/2025. On 5/30/2025, new information was received from the owner about which van he child was left in. When i returned on 6/3/2025 for the continuation of my inspection, the owner stated that she would not be providing any statements without her lawyer present
