A revisit for transportation logs (46-6) was conducted today, 5/10/2023. On today's revisit inspection, the provider is in compliance with standard 46-6 as she is now keeping a record of transportation logs. The transportation logs were not fully complete, therefore, a non- compliance for standard 46-8 is being issued. The logs were missing second person's signature and the dates the transportation log pertains to. Per the logs, the second person did not sign the log for Friday (MM confirmed that this is last week's log 5/1/23 - 5/5/23). Another log (without a date) did not include the driver's signature for Mon- Fri and a the second person's signature for Thu - Fri. Per log, a sweep was not conducted by either person that week on Thursday and Friday. TA was provided that Upon arrival at the destination by vehicle, the driver of the vehicle must mark each child off the log as the child departs the vehicle; conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and record, sign, and date the transportation log immediately. Upon arrival at the destination by vehicle, a second and different staff member must conduct a physical inspection and visual sweep of the vehicle to ensure that no child is left in the vehicle; and sign, date and record the transportation log immediately, verifying that all children were accounted for, and that the log is complete

This standard was non-compliant on previous complaint inspection dated 2/2/2023 as the Owner did not have any transportation logs for the school age children that are transported to the child care center at 5:00PM. On today's revisit inspection, the provider stated that she will not be completing a transportation log for these children being transported because they have nothing to do with her preschool. TA was provided again that a log is required for all children being transported to the child care center. A notice of violation will be issued as this is the second occurrence for this non-compliance. When asked for the transportation logs for the after care children from the public schools, the owner stated that she does not pick-up from the surrounding elementary schools. She was informed that we checked with the surrounding schools and they confirmed that she ,in fact, provides transportation at those schools. TA was provided that if the children being picked up from public schools are being transported to her private school, an aftercare child care license is required at the location. If the children being picked up and brought to the child care center, a transportation log is required

A complaint allegation was received alleging that there are 2 classes for the 2 years old that have 20 children in each class and one teacher per class. Also there are 33 children in the 3 years old class with 1 teacher. A complaint inspection was conducted on 2/2/2023 and ratios were conducted. The naptime ratios were as follows: 1y/o 1:10, Per owner, another teacher was out to lunch. During the course of a complaint inspection, there were a total of 30 3y/o children napping that were split between the classroom and a stage area outside of the classroom enclosed area. Out of the 30, there were approx.. 14 children sleeping in the stage area. The children on the stage were not being directly supervised as the teacher was sitting on a table inside the 3y/o classroom. There is a portable wall blocking the view from the classroom to the stage, making it impossible to directly supervise the children sleeping on the stage

During the course of a complaint inspection, the following was observed unrelated to the complaint; A dog was observed in the lobby of the facility upon arrival. As we walked through the facility, the dog was observed walking around the facility including the 2y/o classroom where children were napping. The owner was informed that no animal may freely roam the indoor/outdoor premises. All pets and animals must be in cages, pens, or on leashes and be protected so they cannot be reached by the children unless the childrens handling of the pet is directly supervised by an adult

A complaint allegation was received on 2/1/2023 alleging that on January 31st, a parent went to pickup her child and the attendant at the preschool was unable to locate him. She glanced in the area where the school bus was parked and notice her child banging on the window to get someones attention to get him out. Per the School owner (DA) this incident did not occur. She denied having any incident where a child may have been left on the bus. Statements were received from 3 school staff at random, all staff denied seeing or hearing of this incident where the child was left on the bus. This complaint could not be substantiated. During the course of a complaint inspection regarding transportation, the transportation logs were requested to check that children are being signed on and off the school bus. The owner (DA) stated that she was unaware that she needed to have a transportation log for the children she brings in from her other school. The children are brought to the preschool at 5:00PM for parents to pick up at this location. The owner denies providing transportation and after care to children from public schools in the surrounding area. On or about 2:00PM, we observed a preschool staff member leave on what appeared to be a 15 passenger white van. At around 2:10 or 2:15PM, QAS contacted two Elementary Public Schools within close proximity of the preschool and they confirmed that Children In The Spirit is one of the transportation companies that picks up children at their school. We observed the vehicle drive back to the school but continued to drive around in the neighborhood with children in the van
