The preschool self reported on 5/5/2025 that a child who got out of the facility through a gate. The child was off campus. FINDINGS: A complaint inspection was conducted on 5/6/2025 and a statement was received from the site supervisor and teacher. Per the director, they had just came in from the playground and were getting ready for nap. The children had set up the sleeping mats in preparation for naptime when the teacher noticed that the child was missing. She stated that they were looking for him when the police brought him in. She stated that the police found him across the street, outside the facility gate. . They had a delivery and the main gate was left open. They do not know how long he had went missing for. Per the director, the police and the parent arrived at around the same time. The parent left before the police left so they didnt have time to write an incident report to give her to sign. Per the teacher, she thinks it happened at around 12pm when they were getting ready for nap. He was helping her put the mats down and when she was ready to turn the lights off she looked for him and didn't see him. She stated that she started looking for him and went to other personnel to see if someone had signed him out. Since he had not been signed out, all the other staff started to help in the search. The complaint is substantiated and a non-compliance and notice of violation is being issued
