Complaint alleges that May 1, 2025, and May 6, 2025, The teachers at Plantation KinderCare failed to provide safety and adequate physical surroundings for all children. During both incidents, the daycare failed to contact the child's legal parents to notify them immediately. On May 1 st ,2025, while running and pushing a bike in the playground, the child sustained an injury to the forehead, resulting in a BIG bump. Teacher A provided the incident report to Aliyahs mother on 5/2/2025. On May 6, 2025, another child pushed her off the slide, resulting in a fracture to her left arm. On May 6, 2025, the teachers B and C supervising the children on the playground had the opportunity to prevent the incident through increased attentiveness to the children's activities on the slide. FINDINGS: A complaint inspection was conducted on (5/19/2025). Statements from the following staff was received: teachers A, B, and C. Per teacher A, she was with the group on the first incident. She saw the child pushing the bike but did not see the child hit the slide structure. She heard the child cry so she went to her and noticed the bump on her head. She said that she can conclude that the child crashed into the play structure because she saw her running toward it and shortly after she heard her crying. Per teachers B and C, teacher B walked to the picnic tables to get a sip of water before the incident occurred so she didn't see the incident. Teacher C was sitting at the picnic tables, tying a child's shoe so she didn't see the incident. The incident happened on the front side of the playground and the picnic tables are on the back side of the playground. Hence, the staff did not see the incident when it occurred. Teacher B heard the child crying and tended to the child after the incident. She asked the child what happened who said that another child pushed her off the gray steps of the slide structure. During today's inspection, the 2 y/o children were on the playground and inadequate supervision was observed. The children were playing on the structure next the front gate and the teachers were in the back near the picnic tables. This complaint is substantiated for inadequate supervision during playground time

Complaint alleges that child had two incidents, one on May 1st and the second on May 6th and parent was not provided an accident/incident report. FINDINGS: A complaint inspection was conducted on (5/19/2025). Statements from the following staff was received: Director, teachers A, B, and C. Per director, she was still on-site when the incident occurred. She believes the accident/incident report was written on the day of but the teacher may have put it in a folder instead of giving it to the parent on the day off. Per teacher A, she stated that she was the one that tended to the child during the May 6th incident. She did not get a chance to write an incident report because the parent arrived shortly after the incident occurred. Per teacher B, she was with the group of children when the incident occurred on May 1st. She doesn't remember if she wrote the report on the day of. She believes that she did. Per the incident report, the reports were given to parent for signature May 2nd and May 7th. Which is the day after each incident. This complaint is substantiated
