During the inspection, Licensing Specialist observed that one of the toilets in the boys bathroom was not operational and there was a hole in the wall in the girls bathroom. During the inspection, Licensing Specialist provided technical assistance, All school-age program facilities must be clean, in good repair, and free from health and safety hazards and vermin. During the hours that the program is in operation, no portion of the building can be used for any activity that presents a risk to the health and safety of the children. Indoor and outdoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Any problems must be corrected before the play area is used by children. Documentation must be maintained for 12 months
