05-04 · Transportation
Transportation monitored Per DCF Handbook:2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each child's name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Transportation By Vehicle E. Prior to transporting children by vehicle, the transportation log must be recorded with each childs name, the date and time of departure and initialed by child care personnel verifying that all children were accounted for and that the log is complete. F. Upon arrival at the destination by vehicle, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and Child Care Facility Handbook Page | 16 3. Record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted. 4. Ensure that a second child care personnel conducts a second visual sweep, signs and dates the transportation log verifying that all children were accounted for, and that the log is complete. 5. If the provider contracts with an outside entity to provide transportation, then the provider must assign a child care personnel to perform the duties of the driver outlined above in numbers 1-3. 04-04 · Supervision
Teacher 1 and Teacher 2 were informed by two children that the child fallen off the merry go round. Neither of the teachers got up from their chairs to check the child for injuries. Licensing Supervisor, Inter Agency personnel and the Owner reviewed the video surveillance to verify supervision requirement were inadequate. The Owner will complete a supervision training for all childcare personnel. The Owner will submit the itinerary and employee sign in roster to the licensing office on or before 03/21/2025 via email to verify compliance. Per Owner, childcare personnel will not be permitted to sit down during outdoor play time. Childcare personnel will be required to walk around the playground to monitor children's play. Per Owner, the facilities cell phone policy states childcare personnel should not have cell phones while in ratio and this was discussed with childcare personnel again

32-12 · Outdoor Equipment
The merry go round has a buildup of sand around the base of the equipment which restricts all movement. According to the product usage the merry go round is supposed to spin when pushed. Licensing counselor and Inter Agency personnel reviewed video surveillance with the Owner and children were observed sitting on the push bars talking to each other instead of using it according to manufacturer. Parts of the push bar have rigid pieces that need to be repaired, sanded down, or something placed on top of it. Per Owner, children will not be permitted to use the merry go round until the sand has been removed from the base and push bar repaired. Per Inter Agency Personnel during an onsite inspection the pool noodles had been placed over the merry go round push bars 02/13/2025

39-01 · Accident/ Incident Notification and Documentation
The child's parent was not informed the child fell backwards off the merry go round and hit her head. After the children had been signed out of the facility, but still in the parking lot the child's mom was informed of the incident by the child's sibling. The child's mom brought the child back in the facility and asked the Owner what happened to the child and showed her the child's head. Per Owner, she had no knowledge of the incident. Per Owner, she talked to the staff and Teacher 1, 2, and 3 stated they had no knowledge of the child being injuring. The Owner completed an incident report after the parent left the facility. Licensing Supervisor, Inter Agency personnel and the Owner reviewed the video surveillance, and it showed the child fall off the merry go round and two children inform Teacher 2 and Teacher 3 the child had fallen off the merry go round. Neither Teacher 2 nor Teacher 3 provided assistance or check the child for injury. The Owner will complete a Supervision and Incident/Accident reporting training for all childcare personnel. The Owner will submit the training itinerary and employee sign in roster to the licensing office via email on or before 03/21/2025

Childcare personnel did not write an incident report prior to the child's mom asking about the injury after pickup. The Owner wrote an incident report after the child and mom had left the childcare facility therefore a signature was not obtained the day the incident occurred. Childcare personnel will ensure all incident/accident are documented on the day it occurs, and signatures obtained by an authorized person at pickup
