05-04 · Transportation
The transportation logs did not have the departure and arrival times listed during the months of April and May (1-16th). Pictures of transportation logs were obtained for the facilities licensing file. The Provider will ensure their transportation logs have all the required components on it and completed correctly when providing transportation services. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. 05-06 · Transportation
During the months of April and May (1-16) a second sweep was not conducted. Per Provider this is due to staffing issues. The Provider will ensure another staff member completes a second sweep at the time of arrival back to the facility after children have exited the bus or van. At the time of inspection there was additional staff in the facility and the Provider stated a second sweep would be conducted from today onward. Licensing counselor discussed options to ensure a second sweep is conducted daily. The Provider will conduct a transportation training for all childcare personnel participating in transportation services. The Provider will submit the training itinerary and staff sign roster to the licensing office on or before 05/31/2024 via email. Per DCF Handbook: 4. Six children under the age of four were observed eating potato chips during lunch time. Licensing counselor discussed this with the cook and childcare personnel. Per DCF Handbook: C. Foods that are associated with young childrens choking incidents must not be served to children under 4 years of age, such as, but not limited to, whole/round hot dogs, popcorn, chips, pretzel nuggets, whole grapes, nuts, cheese cubes/sticks and any food that is of similar shape and size of the trachea/windpipe. Food for infants must be cut into pieces inch or smaller, food for toddlers must be cut into pieces inch or smaller to prevent choking. This applies to all food, even food provided by parents/guardians

3.6 Napping/Sleeping Requirements 3.6.4 Safe Sleep A. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. Counselor conducted inspection during naptime hours and during the inspection counselor noticed an infant asleep in a crib with a receiving blanket placed across the infants back. **Receiving blanket was removed immediately and both the teacher and the director was informed of the receiving blanket being a strangulation/suffocation hazard. **Additionally, before the close of this inspection report, staff have been given a refresher training on safe sleep procedures; per provider

3.8 Fire Safety and Emergency Preparedness 3.8.2 Fire Safety B. Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times. During inspection counselor inspected all fire extinguishers within the facility; all extinguishers were expired with a tag date of April 2022. **Provider submitted through email on 6/5/2023 a copy of the inspection report and pictures of the updated tagged extinguishers, with an up-to-date tag date of June 2023
