Linoleum floor that has missing pieces need to be replaced. Floor also has to be level to prevent tripping hazard. Gate that separates classrooms needs to have a lock installed. All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area.
