The door to the one/two-year-old classroom was not shut completely, a one-year-old pushed the door open and walked outside. The child was unsupervised for 3 minutes. Resolved at time of inspection: The provider had the two teachers in the classroom complete a supervision training. The provider put signs on all doors reminded teachers to make sure they are shut also; a maintenance order was put in to check the doors

2.8 Child Discipline F. The following discipline techniques shall be prohibited in the child care facility: 1. The use of corporal punishment/including, but not limited to: a) Hitting, spanking, shaking, slapping, twisting, pulling, squeezing, or biting; b) Demanding excessive physical exercise, excessive rest, or strenuous or bizarre postures; c) Compelling a child to eat or have in his/her mouth soap, food, spices, or foreign substances; d) Exposing a child to extremes temperature; e) Rough or harsh handling of children, including but not limited to: lifting or jerking by one or both arms; pushing; forcing or restricting movement; lifting or moving by grasping clothing; covering a childs head. *****In camera footage from 3/24/2023, Teacher A was observed to be standing over Student A. Student A swatted at Teacher A with her left hand. Teacher A is then observed to lift her right foot and kick Student A to the floor. Additional footage from this day shows Teacher A grabbing Student A by her right upper arm and then dragging her by that arm across the classroom and depositing her on a nap mat. The teachers employment was terminated on 3/28/2023. In viewing classroom footage from 1/30/2023 and 2/6/2023, the teacher was observed each day to restrain the child and prevent him from playing/moving freely around the classroom for long stretches of time (longer than 4 minutes). In interviews, the teacher stated she did this because she was concerned for the safety of the other children in the class. On both days, there were other staff in the classroom who witnessed this behavior and did not intervene or report it to management. By the deadline, please submit proof that all staff have been trained on appropriate methods of discipline and the facility's discipline policy. Include training materials and a roster in the documentation. From the Licensing Handbook, Section 2.8, D: D. Active play, both indoor and outdoor, must not be completely withheld from children who misbehave.
