Licensing specialist observed one seat belt broken on each vehicle, two broken seat belts in all. Bus #184 has a broken seat belt in the seat directly behind the driver in the spot next to the aisle. Bus #162 has a broken seat belt in the second seat behind the driver in the spot next to the window. Provider taped signs in the two spots stating no children can sit there. Provider will have the seat belts will be fixed by March 3rd, 2025. Per DCF CCF Handbook section 2.5.5: Each child, when transported, must be seated in a back seat in an individual factory installed seat belt or federally approved child safety restraint. The child safety restraint must be installed, secured and used in accordance with the manufacturers instructions

Child care personnel did not complete an accident/incident report when they became aware of the child's injuries. The Provider will ensure accident/incident reports are written on the day the incident occurs. The Provider will train child care personnel on proper documentation for accident/incident reports.
