33-03 · Accident/Incident Notification and Documentation [SR]

The Provider did not complete an incident report the day the incident occurred nor obtain a parent/guardian signature. Per Provider the child's guardian was called and aware of the incident. Per Provider the child has been disenrolled from the program. Licensing counselor discussed documentation requirements with the Supervisor on duty. The Provider will ensure accident/incident reports are completed by childcare personnel and signed by the parent/guardian on the day the accident/incident occurs
