Provider self reported that a child who was attending Silver Shores was transported and dropped off at Sunset Lakes location in error. The staff member then transported the child back to Silver Shores where the child was enrolled, in their own personal vehicle. FINDINGS: An on-site visit was conducted on 7/1/2025 and a statement from the on-site director was received. Per the director, the school age children went on a field trip on Friday. The bus picked up 20 children, 2 staff from Silver Shores and 20 children, 2 staff from Sunset Lakes. Returning from the field trip, the first stop was Silver Shores. The Silver Shores children were sitting in the front section of the bus and the Sunset Lakes children were sitting in the back section of the bus. During the ride back to the school, the child was sitting next to teacher A (BD) on the last row occupied by Sunset Lakes children, near the middle section of the bus. Upon arrival to the school, teacher A (BD) got off and waited for the children in the school lobby. While sitting in the front row, Teacher B (FD) called the children's names so that they could exit the bus in that order. She called out the child's name and she assumed she got out of the bus. The child didn't get off when her name was called because she was asleep. The bus then drove to sunset Lakes to drop off the second group of children. A teacher from sunset lakes noticed the child and notified teacher B via phone call. Teacher B got in her personal car and went to pick up the child from Sunset Lakes. A non-compliance is being issued because the teacher used her personal car to pick up a child enrolled in the preschool without an installed child safety alarm

The child was found awake and she appeared to have been crying because her eyes were red. Teacher B drove to the school, picked up the child and brought her back to Silver Shores. A non-compliance is being issued as the facility failed to verify that all children were accounted for. The log was complete and the child was marked as getting off the bus although she didn't exit the bus
