A complaint was received on 1/5/2026 alleging the following: a child was left in a van for an unknown amount of time on 12/19/2025. The van was off, the child appeared in distress, and the doors were locked. A witness ran into the facility to report this to staff at approximately 12:30-1:00pm and gather assistance with opening the van. The teacher of the child told the child, "why would you not come out with the rest of the group?" rather than owning up to her actions of not conducting a walk through of the van and completing a head count. FINDINGS: A complaint inspection was initiated on 1/6/2026 and statements from the current director (L.A.) and aftercare teacher/assistant director (Y.A.) were received. The driver was not on-site during the initial visit. A follow-up visit was conducted on 1/9/2025 but the driver had not returned to work . Quality Assurance Specialist was unable to obtain a statement from the driver either in person or via phone call. The owner (A.G.) was present on 1/9/2026 so a statement was received and video footage was reviewed. Per the directors (L.A.) interview on 1/6/2026, she didnt know about an incident where a child was left on the van because on the date in question (12/19/2025) she was not the official director. She was only in training at that time but she was working in the office. Per the aftercare teacher/assistant directors (Y.A.) interview on 1/6/2026, a child was not left in the van. There was an incident when a child did not want to get off so the driver was attempting to call her to come help. The driver sat in the van while a woman , who she believed was a passerby, came into the school to tell her that the van driver needed help unloading the children because there was a child that did not want to get off. Y.A. was asked if the driver ever got off the van to which she responded no. She was asked if at any point a child was left alone in the van and she responded no. Per the owners (A.G.) interview on 1/9/2026, he was not made aware of any incident involving a child being left in the van by his employees. Video footage from 12:30PM to 12:42PM on 12/19/25 was reviewed and the following was observed: Between 12:31PM and 12:32PM, the van is observed pulling up to the front of the school. At 12:32PM, the driver is seen getting off the van, walking around the front of the van and proceeding to open side passenger double doors. Seven children were observed getting out of the van. The driver closes the van doors and proceeds to walk into the school with the children. At 12:36PM, a woman (possibly a therapist) is seen walking out of the school. She passed by the front of the van, then walked back to the front where she paused for a moment, and then walked back inside the school., A few moments/seconds later she emerges with another individual who, as per the owner, was the director on the date of the incident. The director (A.P.) and therapist attempt to open the doors but they are locked. The director (A.P.)goes back inside the building. The assistant director (Y.A.) comes outside and the director comes behind her with the keys. The assistant director opens the van door and places one hand on her waist and the other on the van door, appearing to be upset as she exchanges words with the child. The child got off the van on his own and walked inside with no assistance. After further review with the owner (A.G.), we determined that the child that was left alone in the van was O.V. (6 y/o). This complaint is substantiated as the child was left in the van for six minutes. A Class1 Notice of Violation is being issued

During the course of a complaint inspection related to transportation, the following non-compliance was observed; A complaint inspection was initiated on 1/6/2026 and statements from the Director (L.A.) and assistant director (Y.A.)was received. The assistant director was asked if there was an incident that occurred on 12/19/2025 where a child was left in the van. She stated that she does not know the exact date but she was called because the child didn't want to get off the van. She stated that "another lady" came inside to get her. I asked her who the other lady was and if she was employed by the school to which she stated that she does not know the lady and she is not employed by the school. She went outside and the other kids were waiting outside the van and he was still inside the van refusing to get off. The driver was still in the van as well waiting for him to get off. She spoke to him and convinced him to get off. Then, they all came inside as a group. The teacher was asked if she was 100% sure that that day or any other day a child was not left in the van to which she responded yes, she was sure. After reviewing video footage of the incident, there was in fact a child that was left inside the school van. The assistant director was the individual seen on footage that opened the van doors to let the child out on 1/19/2026. A non-compliance is being issued for hindering an inspection by providing false statement to the inspector

ALLEGATION: A complaint was submitted to CCLE on 9/15/25 alleging that a two-year-old child was hit by the teacher towards the lower back. Parent requested to check footage during nap and potty time since the child has shown distress while using the restroom. FINDINGS: An investigation was conducted on 9/16/25 which consisted of interviews with the school administrator and teachers in charge of supervising the two-year-old child. Additionally, footage was reviewed on the last day the child attended the center on 9/10/25. According to the administrator AP, she interviewed both teachers JB and MM and they both stated there have not have any type of incident with the child. Both instructors were asked if any incident occurred on 9/10/25 and they stated nothing unusual occurred that day. According to the teacher JB, the only time the child could interpret "hitting" was the tap on the child's back at the time of nap when she tried to place the child back to sleep. No incident report was documented since nothing occurred under the instructor's care. Facility's owner provided access to the cameras, and footage on 9/10/25 was reviewed during nap and potty time. After the footage was reviewed, the instructor JB was observed supervising children during nap time while the other instructor MM was in her lunch break. At 13:27 the instructor JB approached the two-year-old child and reaccommodated the child in the cot. The footage didn't reveal the instructor hitting the child, however the way she placed the child back to sleep was not appropriate. The instructor was observed forcing children to get back to the cot and placing the blanket on top of the child. Based on the information gathered, the allegation is deemed UNSUBSTANTIATED, however the area was marked as non-compliance due to not following the facility discipline policy. UPDATE: Facility's owner reached out to CCLE via email on 9/17/25 to ask some questions, and a phone call was returned on 9/18/25 to notify that after continuing revising the two-year-old classroom footage, he observed another teacher pulling a child's hair by grabbing a small portion. Provider decided to self-report the incident and stated that the child in question was not the same child from the allegation. Additionally, the instructor has been placed on temporary suspension until further notice. The allegation stands as UNSUBSTANTIATED, and the non-compliance stands for not following the facility discipline policy
