During the course of a complaint inspection, the school was determined to have lacked adequate supervision of a child that received severe chemical burns from a shirt provided by the school. FINDINGS: Per the VPK teachers, the shirt was changed at 10:00AM and the child remained with the shirt on until parent arrived to pick up after 5:00PM. The aftercare teacher stated that the child requested to change her shirt on numerous occasions but the teacher did not investigate further the reason for the child's discomfort. After 4:15PM, the aftercare teacher took the class to the playground. Once in the playground, CB told her several times that she wanted to change her shirt. At that point, the teacher saw the child's chest, below her clavicle, to the right, and a discoloration to her skin caught her attention. She pulled the child's collar open and saw the brown color on the child's skin. She asked the child what happened and the child would not speak because she was crying. The aftercare teacher called the director, who then went to the playground to look at the child and attempted to call the parent. The child remained with the group until parent arrived to pick her up a little after 5PM. Per the accident/incident report, at 2:40PM, the child was crying and when the aftercare teacher asked her "what happened?", the child said she wanted to change her shirt. She didn't have another shirt in her cubby. Later, the teacher noticed something on her neck and reported it to the office. The child's needs were not met in this instance, therefore, causing the child to be exposed to the chemical on the shirt for over 6 hours. A class 1 non-compliance and Notice of Violation is being issued

A complaint allegation was received alleging that on Friday 12/05/2025 at 4:59 PM, the school contacted the mom and said that the child was whining all day after they changed her shirt at around 10am. Parent asked to speak with her daughter and the child said "she had scratched herself. Parent arrived at the school at 5:06 PM. When parent arrived, she noticed that the child's neck and shoulder were darkened. Parent took child to Memorial West at about 5:20 PM. The nurse/doctor told parent it looked like an abrasion or possibly a bruise and that it would go away. However, around 2 AM the next day, the area started to blister. Parent and child returned to Memorial, and this time they said it was a chemical burn. They advised her to go to the daycare on Monday to get an incident report. Parent took child to Joe DiMaggio Childrens Hospital on 12/8/2025 and they diagnosed CB with a third-degree burn and transferred her to Jackson. Parent was told by staff at memorial west that the clothes CB was wearing had chemicals on them. The child told parent that when they changed her clothes, it was itching and hurting all day, and they said she was whining. FINDINGS: A complaint inspection was initiated on 12/11/2025 and completed on 12/15/2025. Statements from the following staff were received: VPK Teachers (C. Coen, E. Medina,) and aftercare teacher (N. Castro). PLEASE REFER TO STATEMENTS ON COMPLAINT FORM. The school is being cited because the child was injured due to a stained shirt that caused a severe burn on her skin. A notice of violation is being issued and the non-compliance is being escalated to a class 1. The incident occurred on 12/5/2025, the investigation started on 12/11/2025, and the Notice of Violation was issued on 12/17/2025

Complainant alleges that on 4/23/2024 the teacher (YB) had left a two year old toddler (CM) alone outside in the hot sun. When parent arrived and asked for the child, the teacher called out his name but he was no where to be found. The parent found the child on the playground alone, with a full diaper eating mulch. An on-site visit was conducted and a statement from the Director (TW) was received. Per the Director, the camera footage was reviewed by the regional manager and herself. The classroom was outside during playground time and when the group came inside, they left CM on the playground. The teacher failed to do face-to-name as they entered the classroom and only counted heads. It was determined that when the parent arrived to pick up the child at 4:35PM, the child had been left alone in the playground for about 8 minutes. A statement from the teacher, YB, was not received as she was terminated following an internal investigation. This complaint is Substantiated and a notice of violation is being issued. The section number cited in the inspection report is incorrect. The facility is being issued a Notice of Violation for violating Section 7-33 (e),Ordinance
