Upon arriving to the program, a crane was observed in front of the facility placing an AC unit on top of the facilitys roof. The entrance/exit and indoor area directly under the placement of the AC unit were not secured for children present at the time of the inspection. Hazardous equipment/tools were being used while children attending the program. Provider decided to stop conducting work in the roof area while children present. Therefore, this area was brought back into compliance during todays visit

During the course of a complaint inspection where a child (IQ) received an injury to his left leg, the video of the incident was reviewed and the following was observed; Video footage of the incident was observed and date and time as described on the video name was 9/6/2024 at 8:40AM. The child was observed walking by the bookshelf. He tried to take a side step and put his foot on top of a book that was laying on the floor. The book slid the opposite direction of his body causing him to do a split. He started crying and repositioned his body into a sitting position. Two staff members were observed in the room. One staff member had her back toward the child and the other had her side toward the child. None of them saw what happened and one of them (Ms. JC) turned around to assist the child. She bent down, hugged him, picked him and took him away from camera view. Although there were two staff members in the classroom, Inadequate supervision occurred in that the staff members were observed standing together and appeared to be engaging in a conversation with someone off camera view not watching and directing the children's activities
