During the inspection, the Licensing Specialist observed indoor play areas were not in good repair in that the tic tac toe toy connected to the jungle gym was cracked and the bottom area had sharp edges accessible to children in care. There was black substance throughout the surfaces of the jungle gym. The Operator has been given until 7/4/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 7.6, A: Outdoor Play areas shall be clean, in good repair and free from litter, nails, glass, and other hazards

REMINDER: 10 hours of annual in-service training must be completed by the license expiration, 6/11/2025. During the inspection, Licensing Specialist observed that the substitute card/certificate did not state that the skills were met. During the inspection, Licensing Specialist provided technical assistance, Have certificate(s) of course completion for pediatric cardiopulmonary resuscitation (CPR) procedures and first aid training, which must be current and valid at all times. Certificates of course completion are valid based on the time frames established by each first aid and CPR training program, not to exceed three years. CPR courses must include an on-site instructor-based skills assessment that shall be documented by the certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the home and available for review by the licensing authority

At the time of inspection, licensing specialist observed and photographed cleaning supplies consisting of Lysol spray, a perfume body spray and Mr Clean magic erasers on low shelves in the children's restroom. All supplies were moved to a higher shelf out of the reach of children. At the time of inspection licensing counselor observed the fence having a gap open at the bottom of the fence. Photos were taken. The opposite side of the fence had the same issue but cement blocks were laid to secure the opening so no small animals could crawl in under the opening. Be sure to secure the fence at the bottom by due date. TA per family home daycre handbook, the bottom or base of the fence must remain at ground level and free from erosion or buildup to prevent inside or outside access by children or animals
